Qualifications:

High school diploma or equivalent; must have phone accessibility, valid driver’s license and adequate transportation on-site and available to transport individuals; ability to provide support services in the home and community for adults with intellectual/ developmental disabilities, brain injuries and various medical/clinical conditions; Ability and willingness to demonstrate compassion, positivity, reliability, be team-oriented, participate in required training and team meetings; Ability to provide total care and physical assistance for individuals who require this level of support; Ability to provide support for individuals who may have mental health or behavioral challenges; ability to demonstrate positive social skills, courtesy, respect, professional boundaries and serve as a role model for individuals and co-workers; ability to demonstrate effective verbal and written skills.

Responsibilities:

  • Assist individuals with participation in recreational, social, community, spiritual activities within their respective home and community, based on individual choice and interest;
  • Assist individuals, as indicated, in all aspects of activities of daily living including areas of:

Health, hygiene, bathroom/toilet

Hoyer lift and transfers from wheelchairs

Changing garments and laundry

Meal preparation and shopping

Budgeting and finance

Bedroom and home cleanliness and exterior maintenance of home

  • Provide support to individuals to develop and sustain meaningful relationships with family, friends, co-workers, and neighbors;
  • Ensure each individual’s home offers safety, rest, satisfaction and enables individuals to have choice and control and feel at ease;
  • Participate as an active, positive member of the CMHS team, including, but not limited to, consistent attendance in team/house meeting and in-service training;
  • Understand, follow and adhere to company and site policy and procedure, related documentation, and clinical/medical protocols;
  • Consistently promote wellness-related activities in the home and community;
  • Acquire and maintain all program-mandated and regulatory training requirements
  • Conduct and supervise fire safety drills;
  • Other related duties as assigned.
